PSV Operator Forms
Introduction
Similar to HGV operators, there are many forms that are required and available to the PSV operator. There is a full list of all the forms
required below, with a brief overview of what they're required for and the relevant link to download from within this website. All the
forms below are listed in their numerical order and are downloadable in either word or pdf format.

This form concerns the application of individuals/company's who are wish to apply for a Hackney Carraige License. Under Section
12(1)(b) of the Transport Act 1985 requires all applicants for special public service vehicle operators licences to state in their
application that they propose to provide a local service. A declaration is required in respect of this.

This form should be completed in conjunction with "A Guide to Public Service Vehicle Operator Licensing", which will also assist
in answering many questions as well as completing other forms found here.

This form is for those operators who have applied for a PSV operators license and upon its grant will require to supply the authorities
with the vehicle registration marks and types of vehicle(s) to be used.

This form concerns supplementary financial information required, which is additional to that financial information submitted upon the
orginal license application (PSV 421). The information required here is an in depth overview of your source of finance, operating and
running costs.

This form concerns any vehicle changes to your fleet which will affect your operator license details, namely vehicles disposed of and
acquired.
|
|
This form concerns any variations required to be made to your existing operators license, namely an increase in the maximum number of
vehicles to your license and/or the upgrading of your license from a standard national to an international license. This varaition to your
license is chargeable.

This form concerns any variations required to be made to your existing operators license, namely a reduction in the maximum number of
vehicles, variations to any attached conditions to your license, change of Transport Manager, Operating Centre or maintenance provider. This
form will be required alongside form PSV 430 if additional vehicles of a type previously restricted are to be added.

This booklet must be considered the PSV operators bible, inasmuch as it holds all key information regarding the completion of the forms
available for download on this page as well as information on other legislation pertaining to PSV operations.

This form is to be used to obtain replacement operator license vehicle disc(s), where the original(s) of which has/have been lost or
destroyed.
